Why Stretch Marks Form, How Red Lines Turn White, and When Treatment Works Best

Look at the lines running across your belly, thighs, or upper arms, and it is hard to tell right away when they showed up. That is because they usually start out red or purplish, then quietly fade into pale, silvery streaks over time. Most people never realize that color shift is not just fading. It is a sign that the tissue underneath is genuinely changing.
Stretch marks show up when skin stretches too far, too fast, in too short a window. That is exactly why they tend to appear during growth spurts, pregnancy, rapid weight changes, and periods of fast muscle gain. Here is a closer look at why they take that streaky shape, why the color changes, and when treatment tends to work best.
Why Does Skin Tear When It Stretches?
Skin has two main layers, the epidermis on top and the dermis underneath, and stretch marks form in the dermis. Inside the dermis, collagen and elastin fibers weave together like a net. Think of collagen as the support pillars holding your skin up, and elastin as the rubber bands stretching between them.
When skin stretches slowly, those pillars and rubber bands have time to shift and rearrange themselves without snapping. The trouble starts when the stretching happens too fast. During the later stages of pregnancy, the belly can noticeably expand within an 8 to 12 week window, and rapid weight or muscle gain creates the same kind of pressure. The collagen pillars simply do not get enough time to reposition, so they stay pulled taut until they tear.
Once a pillar tears, the gap does not heal back into smooth, normal tissue. Instead, the body patches it quickly with thin, weaker fibrous tissue. That patch job ends up thinner and differently textured than the skin around it, and that is the streak you see as a stretch mark.
Why Do Stretch Marks Show Up Most on the Belly and Thighs?
Stretch marks are not spread evenly across the body. They cluster on the belly, thighs, hips, and chest, and there are two reasons for that.
The first is simple physics. These are the spots that stretch the fastest and the most. The belly expands significantly during that same 8 to 12 week stretch in late pregnancy, while the thighs and hips are usually where fat and muscle build up first during growth spurts or weight changes.
The second reason is hormonal. Pregnancy and growth spurts both come with a rise in cortisol, and cortisol has a way of quieting down fibroblasts, the cells responsible for producing collagen. Much like a person tensing up under stress, collagen production slows down under cortisol's influence. So at the exact moment skin needs to reinforce itself the fastest, collagen renewal actually slows, which is why these areas are more prone to tearing than anywhere else.
What Is the Difference Between Red Lines and White Lines?
The reason a fresh stretch mark looks red or purple comes down to simple biology. When collagen tears in the dermis, it triggers an inflammatory response, and that draws in more blood flow through widened vessels near the surface. What you are actually seeing is blood vessels showing through thinned skin, which is why this early stage is called striae rubra. Rubra is Latin for red.
Over time, that same area gradually fades to white. As the initial inflammation settles down, blood flow decreases, and the tissue that patched the tear stabilizes into fibrous tissue with lower collagen density and less pigment. This later stage is called striae alba, or white stretch marks. Alba is Latin for white.
In other words, the shift from red to white is not an optical illusion of fading color. It is a real structural change, as inflammation calms down and the tissue hardens into scar-like fibrous tissue. This transition usually unfolds over 6 to 12 months, and can take as long as 2 years in some cases.
Why Do Stretch Marks Feel Slightly Sunken to the Touch?
Run your fingers over a stretch mark, and you will often notice it feels a bit indented compared to the skin around it. That texture is not a coincidence either. It reflects a real change in the structure of the dermis.
Normal skin has a dense web of collagen pillars in the dermis that firmly supports the epidermis from below. But in a stretch mark, those pillars snapped and got patched with sparse fibrous tissue in a hurry, so there is less support holding the surface up, and it settles slightly. The area is also genuinely thinner than surrounding skin, which is why it feels different in texture and slightly pressed in under your fingertips.
That is exactly why stretch mark treatment focuses on rebuilding thickness, not just fading color. Even if the color lightens, a mark that still feels sunken tends to leave people unsatisfied with the result.
Why Does Treatment Work Better While Marks Are Still Red?
There is a real biological reason treatment tends to work better during the red phase. Red stretch marks are still actively inflamed, with blood vessels actively engaged, and the fibroblasts responsible for making new collagen tend to be more active during this window as well.
Treating skin during this phase tends to push those already-activated fibroblasts to ramp up collagen production even further. Once a mark has fully hardened into white striae, blood vessels have thinned out and the tissue has become fibrous, and studies suggest cells respond more slowly to the same kind of stimulation at that stage.
That does not mean white stretch marks are untreatable. But clinical experience consistently shows that starting treatment during the red phase tends to produce more noticeable improvement than waiting until marks have fully turned white, which is why getting a consultation early is generally recommended.
How Do Lasers Actually Improve Stretch Marks?
Lasers used for stretch marks generally fall into two categories. One targets blood vessels directly, and the other stimulates the skin tissue itself.
Vascular lasers are typically used on red stretch marks, and the principle is straightforward. Laser light gets selectively absorbed by hemoglobin inside blood vessels, and the resulting heat shrinks those widened vessels. As the vessels causing the red appearance settle down, the redness fades.
Fractional lasers, on the other hand, focus less on color and more on rebuilding thickness and texture. The laser creates tiny, controlled zones of micro-injury across the skin, which triggers a healing response as the skin produces fresh collagen and elastin to repair those spots. The word fractional itself refers to treating small, scattered fractions of the skin rather than the whole surface at once. Through this repair process, thinned stretch mark tissue has been reported to gradually thicken and smooth out over time.
Red stretch marks are often treated with a combination of vascular and fractional lasers, while white stretch marks are usually treated mainly with fractional lasers, since the focus shifts entirely to rebuilding thickness and texture.
What Else Should You Consider When Timing Treatment?
When you are weighing stretch mark treatment, it helps to look beyond just color and consider a few other factors together.
- How long the stretch marks have been there matters. Since treatment tends to respond better during the red phase, knowing roughly when the marks appeared is the first thing to factor into a treatment plan.
- Skin thickness at the site matters too. Thicker areas like the belly and thighs respond differently than thinner areas like the underarms or chest, so results may show up on different timelines even with the same treatment.
- Pregnancy or breastfeeding calls for adjusting the timing. The bigger concern is usually not that the laser itself poses a direct risk to a fetus or infant, but that skin is still actively changing during this period, which makes it hard to properly judge how well treatment is working.
- Whether your skin tends to form keloid scars is also worth checking ahead of time. In that case, laser stimulation can potentially trigger other kinds of scarring, so it is something to bring up during a pre-treatment consultation without fail.
Can Stretch Marks Be Prevented?
Since genetics plays a major role in how elastic your collagen naturally is, stretch marks cannot be completely prevented. That said, managing how fast your skin stretches is reported to help.
Pacing weight gain during pregnancy so it does not spike too quickly, or building muscle gradually instead of pushing for rapid gains, are both good examples of this approach. The slower skin stretches, the more time collagen pillars have to reposition themselves, so gradual change over sudden change is generally reported to lower the risk of developing stretch marks.
It is fair to wonder whether moisturizers or stretch mark creams actually help prevent them. Creams themselves do not make collagen pillars any stronger. But dry skin tends to lose elasticity and micro-tear more easily under the same amount of tension, so keeping the skin surface supple with consistent moisture is known to help. In short, think of cream less as a shield that blocks stretch marks outright, and more as a supporting player that slightly reduces the conditions that make skin prone to tearing.
