Why Vertical Neck Bands Form From Platysma Muscle Contraction, and How Botox Relaxes Them

When you look at your neck in the mirror, it is not always the horizontal creases that catch your eye. Sometimes it is a pair of vertical lines running from under the jaw down toward the collarbone. They tend to stand out more when you smile, talk, or tilt your head down. People often call these neck bands, and the cause is a bit different from sagging skin.
The source of these vertical lines is a thin muscle that spreads across the front of the neck. Here is why this muscle tends to show up as raised lines, and how Botox acts on it to soften them.
What Does the Platysma Muscle Do?
At the front of the neck sits a muscle called the platysma. Rather than a thick muscle anchored to bone that generates strong force, it is thin and broad, similar to the muscles that shape facial expressions. It starts around the collarbone and upper chest, runs up past the jawline, and reaches the corners of the mouth.
Because it is so thin, it is closer to a latex glove stretched over the entire neck than a typical muscle. It contracts along with facial expressions, especially when you clench your jaw or make a startled face. The moment your neck looks like cords are standing out from anger, this muscle is actually the one pulling.
Since it contracts with nearly every expression, it tightens and releases dozens of times a day. It does not move bones or joints, its only job is to pull on the skin, and that thin structure sitting right under the skin is what sets the stage for vertical bands to form.
How the Vertical Bands Form
The platysma looks like a single broad sheet, but it is actually made up of several muscle bundles that sit side by side. In young, elastic skin, the space between these bundles is smoothly covered, so they do not show from the outside.
As we age, though, the elasticity of the skin and connective tissue underneath declines, and the coverage over the gaps between bundles weakens. Each bundle then stands out on its own whenever the muscle contracts, appearing as a vertical line. It is similar to a stocking that has lost its stretch, letting the tendons underneath show through.
This muscle also often runs as a pair, one bundle on each side of the midline, so it commonly shows up as two separate lines rather than one. Every time the muscle moves, these two lines fold and unfold repeatedly, and over time the crease becomes more defined.
How Botox Relaxes the Muscle
Botox is made from a substance called botulinum toxin, and it works by temporarily blocking the signal that nerves send telling a muscle to contract. At the junction where nerve meets muscle, it reduces the release of the chemical messenger that carries that signal, so the muscle cannot contract as forcefully.
Think of it like dimming a light switch rather than flipping it off. The muscle is not shut down completely, its contraction strength is simply turned down. When the platysma's pulling force is reduced this way, the vertical bands that used to stand out each time the muscle tightened become less pronounced too.
This is also why Botox is injected at multiple points rather than one. Since the muscle is split into several bundles, small amounts need to be placed along the path of each bundle so the strength is reduced evenly across the whole muscle. If injections are concentrated in just one spot, the other bundles keep contracting at full strength and the lines will not fade evenly.
Why Botox Is Often Considered Before Filler for Neck Lines
Neck wrinkles fall into two categories, vertical bands and horizontal creases, and they come from different causes. Vertical bands, as described above, form from repeated muscle contraction. Horizontal creases form when the skin itself folds or loses volume.
Because of this, Botox, which reduces muscle strength, addresses the root cause of vertical bands directly. Horizontal creases, on the other hand, are not a muscle problem but a matter of skin being pressed and folded, so reducing muscle strength with Botox does little to change them. In these cases, filler or treatments that improve skin elasticity fit the cause better.
This is why the approach differs depending on whether it is a band or a fold, even though both are called neck wrinkles. The first step is figuring out whether the line is standing vertically or pressed in horizontally.
How Long Results Take to Appear and Last
Botox works by gradually blocking the signal between nerve and muscle, so results do not appear instantly. The muscle's strength starts to decrease gradually over the first few days after treatment, and the effect is generally reported to be most noticeable one to two weeks in.
After that, the blocked nerve endings slowly regrow and signal transmission gradually recovers. The muscle regains its strength, and the vertical bands that had softened begin to reappear little by little. That is why many people consider a follow-up treatment before the muscle's strength fully returns.
How long the results last depends on the thickness of the muscle, the dose injected, and each person's metabolic rate. Thicker, more active muscles tend to see nerve function recover faster, so the duration people experience can vary quite a bit.
If you tend to talk a lot or make expressive facial movements, you are effectively moving this muscle more often, so nerve recovery may feel relatively faster. Conversely, if you rarely move your neck dramatically, the same dose may last a bit longer.
What to Consider Before and After Treatment
Because the platysma is thin and spreads over a wide area, it calls for a more careful approach than other muscle treatments. The following points are worth keeping in mind.
- Check the number and location of bands first. Since the position and number of raised muscle bundles differ from person to person, injecting only at fixed points without checking beforehand can leave some bands untreated.
- Divide the dose carefully. Deeper muscles involved in swallowing sit just beneath the platysma, so injecting too much or too deep can affect them as well.
- Assess the degree of skin sagging too. If the skin itself has loosened considerably, separate from the muscle issue, Botox alone may not soften the bands enough, and other options may need to be considered alongside it.
- Avoid rubbing or pressing hard on the neck right after treatment. If the product has not yet settled and spreads to surrounding muscles, it can weaken strength in areas beyond the intended spot.
What Should You Watch Out for Afterward?
For the first few hours after treatment, it is recommended to avoid lying down or bending your head forward sharply. This lowers the chance that gravity causes the product to spread in an unintended direction.
It is also best to avoid saunas or hot baths on the day of treatment. Heat increases blood flow, which can cause the product to spread over a wider area than intended. For the same reason, it is safer to hold off on intense exercise for a few days.
Some swelling or light bruising can occur at the injection sites, and this is generally reported to resolve within a few days. Since the neck keeps moving with every expression, it is best during recovery to move naturally as you normally would rather than pushing through strenuous muscle activity.
It is also best to avoid tilting your head sharply backward or resting heavy objects on your neck for a few days. Placing strain on an area that has not yet settled can interfere with the product taking hold properly.
