How Masseter Botox Slims a Square Jaw by Shrinking the Chewing Muscle, and How Long the Effect Lasts

Some people catch their reflection and feel their jawline looks square and thick. This is especially true when clenching the teeth makes both sides bulge out under the jaw. In cases like this, the cause is more likely to be muscle than bone. When the muscle used for chewing is overdeveloped, the lower face reads wider and more angular.
This is where masseter Botox comes in. The name is familiar, but many people do not actually know what happens inside the muscle to make the jawline slimmer. Below is an easy walk-through of why the muscle weakens, how the slimming process unfolds, why the effect does not appear right away, and how long it lasts.
Why might a square jaw come from muscle rather than bone?
There are largely two reasons a jaw looks angular. One is that the lower jawbone itself is wide and angular. The other is that the muscle covering the bone is thick enough to make the angle stand out. Masseter Botox helps with the latter, when muscle is the cause.
This muscle is called the masseter (the muscle used for chewing, running from below the ear to the corner of the jaw). It contracts strongly when chewing food or clenching the teeth, so people who habitually clench their teeth or often eat hard foods tend to develop this muscle the way a forearm thickens with repeated exercise.
If the area under the jaw feels firm and bulges when you clench your teeth, muscle is likely playing a role. On the other hand, if clenching produces little change, bone structure may be the main cause, in which case the change you can expect from Botox is limited. That is why it matters to check this difference before considering treatment.
How does the toxin weaken the chewing muscle?
For a muscle to move, a nerve has to send it a signal. The substance that carries this signal is acetylcholine, think of it as the message a nerve sends telling the muscle, contract now. The point where nerve meets muscle is called the neuromuscular junction, and this is the mailbox where that message gets delivered.
Botulinum toxin, commonly known as Botox, is a substance that blocks the message from leaving this mailbox. It blocks the process by which acetylcholine is released from the nerve ending itself, so no matter how much the nerve tries to signal, it cannot get through to the muscle. Rather than cutting or removing the muscle directly, it works by cutting off the instructions heading to it.
When injected into the masseter, the contraction instructions reaching this muscle decrease. Clenching the teeth no longer bunches the muscle up with the same force as before, and chewing strength itself temporarily weakens. This does not mean the ability to chew disappears entirely, it is better understood as excess force being dialed down to a moderate level.
How does the muscle shrink and the jawline narrow over time?
A muscle that is not receiving instructions works less, and a muscle that works less gradually decreases in size over time. This is called muscle atrophy (a phenomenon in which a muscle gradually loses bulk from disuse). It is the same principle behind an arm becoming thinner after being in a cast for a long time.
The masseter is used every day for chewing and speaking, so it never fully rests, but as it continues to move with reduced strength, the muscle fibers gradually thin out. As the muscle thins, the layer covering the bone becomes slimmer, so from the side, the edge that used to stick out under the jaw softens into a smoother line.
Because this process is a physical reduction in muscle bulk itself, the approach is different from treatments that tighten skin or remove fat. Since the bone and fat are left untouched and only the muscle thickness decreases, the change tends to be more noticeable the more the muscle originally contributed to the jawline.
Why does the effect show up gradually instead of right away?
Right after treatment, there is no major change in appearance. It takes 3 to 7 days for the nerve-blocking effect to fully set in, and the muscle bulk actually shrinking takes even longer. Most people first notice weaker chewing strength within 1 to 2 weeks after treatment, while the slimmer jawline usually does not become noticeable until around 4 to 6 weeks.
This time lag comes from the pace at which the muscle atrophies. The nerve signal is blocked relatively quickly, but for the muscle fibers to actually thin out, changes have to accumulate at the cellular level. It helps to think of it like a tree branch after pruning, it does not thin out immediately, it dries out and narrows gradually over time.
So there is no need to feel impatient if no noticeable change has appeared 2 to 3 weeks after treatment. The maximum effect is usually reported to appear between 8 and 12 weeks, and this is generally the point at which the final change in the jawline is checked.
What determines how much the muscle actually shrinks?
Even with the same dose, how much the jaw slims down varies from person to person. The factors behind this difference are as follows.
- Original muscle thickness. If you habitually clench your teeth or often chew hard foods and the muscle is well developed, there is more room for it to shrink, so the change tends to be more noticeable.
- The ratio of bone to muscle. If bone accounts for a large share of why the jaw looks angular, the bone contour itself remains even after the muscle shrinks, so the change may not be as large as expected.
- Chewing habits after treatment. If you keep enjoying hard foods or gum, the remaining muscle keeps using force in response to that stimulation, which can slow or reduce the extent of atrophy.
For this reason, it matters for the provider to have you clench your jaw during the consultation to directly check the thickness and extent of the muscle. Gauging how much the muscle contributes makes it possible to realistically predict the extent of change you can expect.
How long do results last, and when would a touch-up be needed?
The reduction in muscle strength from Botox is not permanent. Over time, new branches grow from the nerve ending and begin delivering signals to the muscle again. It is a process similar to a new pathway opening up in a mailbox that had been blocked. Once the signal returns, the muscle begins to work again and tends to gradually return toward its original thickness.
The effect of a first treatment is generally reported to last between 4 and 6 months. However, this period varies from person to person depending on the dose used, the original size of the muscle, and everyday chewing habits. Interestingly, with repeated treatments, the degree to which the muscle recovers tends to decrease little by little, so the interval between touch-ups is reported to become longer than the first time. This is thought to happen because there is a limit to how much a muscle that has gone unused can grow back.
If you want to maintain the line long-term, repeat treatment is usually considered once the effect starts to fade. Rather than repeating the same dose every time from the start, it is generally advised to check how much the muscle has recovered each time and adjust only as much as needed, as the safer approach.
What is good to know before and after treatment?
There are a few points worth keeping in mind when considering this treatment.
- Chewing strength decreases along with the slimming. Even though the goal is jawline shape, the masseter is a muscle responsible for chewing function, so right after treatment, you may feel a bit of strain when chewing tough foods.
- It takes time for the effect to fully settle in. Rather than judging the result at 2 weeks, it is more accurate to check the outcome based on how things look at 8 to 12 weeks.
- If bone accounts for a large share of the cause, it helps to gauge the extent of change you can expect beforehand. Since the bone contour remains even after the muscle shrinks, it is necessary to check the proportion of the cause during the consultation and set realistic goals.
Knowing these points in advance helps narrow the gap between expectations and the actual result after treatment. Masseter Botox can be summarized as a method that, when muscle rather than bone is the cause of a square jaw, temporarily reduces the nerve signal to the chewing muscle to naturally bring down muscle thickness.
