Why Fractional CO2 Laser Micro-Wounds Fill In Scars and Pores, and How Long Recovery Takes

If you have dealt with acne for years, or your pores just will not stop looking large, there is a name that comes up again and again at the dermatologist's office. Fractional carbon dioxide, or CO2, laser. The name alone makes it sound like the whole face gets torched, but that is not actually what happens.
Instead of treating the entire surface, this laser drills tiny, precise columns into just a fraction of the skin. As those columns heal, fresh collagen fills in, and scars and pores gradually shallow out. Here is why the laser works this way, how those tiny wounds end up filling in your skin, and how long you can expect to wait for results.
Why does CO2 laser work so well on skin?
CO2 laser light has one defining trait, it gets absorbed by water almost instantly. Since skin is mostly water, aiming this light at it makes the water in that spot flash into vapor in a fraction of a second. Think of it as an extremely precise heat stamp.
That heat vaporizes the skin's surface and the shallow layer just beneath it, taking old or damaged tissue with it. The key is that the laser can control exactly how deep that heat goes. It damages only as far down as needed and stops there, which makes it well suited for problems like scarring that sit deep in the skin.
Why tiny dots instead of the whole surface?
Older CO2 lasers used to blast the entire skin surface evenly. It worked, but treating the whole face at once meant a long recovery and real risk of infection or pigmentation problems.
The word fractional means treating a fraction of the surface rather than all of it. A fractional laser splits the beam into a grid of very fine dots, like a stamp made of tiny needles instead of one solid surface. Picture pressing hundreds of pinpricks into the skin rather than stamping the whole area flat. That lets the laser stimulate the skin broadly while leaving most of it untouched, so you keep the effect but cut down the toll it takes on your skin.
Why does a tiny burn trigger new collagen?
Each of these micro-dots is called a microthermal zone, essentially a pinpoint burn. To your body, it registers as an injury, and injuries kick off repair mode right away.
Blood flow rushes to the site, and fibroblasts, the cells living in the dermis (the collagen-rich layer beneath your skin's surface), spring into action. Fibroblasts are basically tiny collagen factories. They normally work at a steady, slow pace, but the moment they sense damage, they speed up and start pumping out collagen faster and in larger amounts. In effect, the micro-wounds the laser creates flip the switch that restarts collagen production in the dermis.
Why does leaving healthy tissue behind speed up recovery?
Another advantage of the dot-by-dot approach is that untouched, healthy skin stays intact between the treated spots. Unlike a full-surface treatment, the area right next to each wound is still packed with intact cells, blood vessels, and hair follicles.
That surrounding healthy tissue becomes a shortcut for healing. During re-epithelialization, the process where fresh skin cells migrate in to cover the wound, nearby healthy cells travel quickly to close the gap. If the whole surface were damaged, those cells would have farther to travel and there would be fewer of them, so recovery would drag on. But when healthy tissue sits close by, even in narrow gaps, there are simply more cells available to help, so healing moves faster.
How do these micro-wounds actually fill in scars and pores?
Acne scars and enlarged pores usually come down to the same root cause, a lack of collagen leaving the area sunken. Scars form where collagen never fully rebuilt after inflammation, and pores often widen because the collagen and elastic tissue that used to support the follicle wall have weakened, letting the pore stretch out.
As explained above, the laser's micro-wounds trigger new collagen production right beneath the surface. That new collagen settles in gradually over 3 to 6 months, filling in the sunken scar bed and propping up the stretched pore wall from underneath. Scars do not vanish completely, but reports show the depth shallows out and the edges soften as collagen fills in. Pores are said to see a similar tightening effect once the wall regains its support.
Why does overall skin texture improve too?
Fractional CO2 laser affects more than just visible scars and pores, it changes the overall texture of your skin. There are two reasons for that.
First, the laser sweeps away surface debris and old cells along with the damaged tissue, so the new cell layer that comes in sits smoother and more even. Second, elastin, the fiber responsible for skin's bounce, gets stimulated alongside collagen during this repair process and is reported to strengthen gradually as well. Together, these two effects usually show up as smoother texture and a more even tone across the treated area.
How long does recovery actually take?
Because fractional CO2 laser genuinely creates micro-wounds on the skin's surface, it does need real recovery time. Right after treatment, skin turns red and swells, and it is common for tiny scabs or flaking to appear on the surface somewhere between 2 and 4 days later.
It is important to let those scabs fall off naturally rather than picking at them, since fresh skin is already forming underneath through re-epithelialization. Depending on the intensity, the redness typically fades within 1 to 2 weeks, while the deeper collagen rebuilding continues quietly for 3 to 6 months, sometimes stretching out to 12 months. Strict sun protection and moisturizing during this window are said to affect both how fast you heal and how the results turn out.
How many sessions does it take to see results?
Since new collagen takes time to form and settle, one session alone rarely delivers dramatic change. That is why the typical protocol spaces sessions 4 to 8 weeks apart, running 3 to 5 sessions in total.
The spacing exists because of that same collagen production cycle. Giving the dermis enough time to build and settle new collagen from the last session before delivering the next stimulus keeps the skin from being overwhelmed while still letting the results build up over time. Depending on how severe the scarring or pores are, going through 3 to 5 rounds while watching gradual improvement is generally how this plays out.
What to watch during recovery
- Apply sunscreen generously. Skin that is actively regenerating is especially prone to pigmentation, so sun exposure during this window can actually darken the treated area.
- Do not pick at scabs or flaking skin. Re-epithelialization is still underway underneath, and forcing a scab off can tear away the fresh skin beneath it, raising the risk of scarring.
- Moisturize more than you normally would. The skin's protective barrier is temporarily weaker, so it loses moisture faster and becomes more reactive to irritation.
- Skip intense workouts and saunas right after treatment. Raising your body temperature and sweating can amplify the inflammatory response in skin that is still healing.
