Look closely at your forehead or temples in the mirror, and you might notice small, yellowish bumps, like tiny grains, dotting the skin. Squeezing them brings up nothing, and even after 2 to 4 weeks of spot cream, they just sit there unchanged.
Most people assume this is acne, but more often than not, it's sebaceous hyperplasia, a condition where the oil glands themselves simply grow larger. Let's walk through why it shows up in that exact spot and takes that particular shape, how to tell it apart from acne, and how to clear it.
What Do Sebaceous Glands Actually Do?
Every hair follicle in your skin has a small gland attached to it called a sebaceous gland. Its job is to produce sebum, an oily substance, and release it through the follicle onto the skin's surface. Think of it as a tiny lubrication factory, coating the skin to lock in moisture and keep it from cracking.
The forehead, nose, and cheeks, commonly called the T-zone, carry more and larger sebaceous glands than the rest of the face, and that's exactly where sebaceous hyperplasia tends to appear.
That your face doesn't feel painfully tight after washing is thanks to this oil. Areas with almost no sebum, by contrast, tend to crack in winter, which shows just how quietly hardworking each gland is, every single day.
Why Does an Enlarged Gland Turn Into a Raised Bump?
Like any other cell, sebaceous gland cells are naturally replaced over time. But as we age, that turnover slows down, and small lobules, tiny sac-like clusters that look a bit like a bunch of grapes, start forming around a single gland and piling up.
As each lobule swells like a balloon and their numbers grow, the whole cluster pushes up against the skin's surface as a raised, yellowish bump. Because it's the gland tissue simply increasing in volume, it's called hyperplasia, and it's a benign change with nothing to do with cancer.
Picture inflating a single balloon versus tying a bundle of balloons together and inflating them all at once. As more lobules join the cluster, the bundle thickens and the bump becomes more noticeable, so something that started out small can feel noticeably bigger after 5 to 10 years.
Why Is There a Dimple Right in the Center?
Look closely and you'll notice a small dent right in the middle of the bump. That spot is where the follicle opening used to be, the original exit route for sebum.
As the lobules swell up around this opening, they grow in a ring, like a donut, leaving the center hollow. That's why the middle sits noticeably lower than the surrounding tissue. This central dimple is one of the clearest giveaways of sebaceous hyperplasia.
It's a bit like bundling several straws together: the hole in the middle stays open while the bundle around it thickens. Press gently with a fingertip and you can feel the edges bulge while the very center dips slightly, and often you can see it clearly in bright light without even needing a magnifying glass.
How Is It Different From Acne?
Acne bumps also feel small and raised, which is exactly why the two get confused so often. But they start from completely different causes. Acne is an inflammatory reaction, where a clogged follicle lets bacteria grow and the skin swells and reddens. Sebaceous hyperplasia, on the other hand, is the gland itself slowly growing larger, with no inflammation involved. Three features tell them apart.
- Color and redness: Acne turns red from inflammation, while sebaceous hyperplasia stays yellowish or flesh-toned and rarely reddens. Whether inflammation is present is what decides the color.
- The center: Acne tends to have a raised or pus-filled peak, while sebaceous hyperplasia has a dimple in the middle. The two grow around the follicle opening in entirely different ways.
- What happens when you squeeze: Acne releases oil or pus when squeezed, but sebaceous hyperplasia yields nothing at all. There's nothing pooled inside to press out, since it's the gland tissue itself that's enlarged.
Checking just these three points usually makes it easy to tell the two apart. Say you assumed it was a pimple and waited 1 to 2 weeks, but the redness never faded and squeezing brought up nothing, that points toward sebaceous hyperplasia rather than acne.
Why Does It Favor Oily Skin After Midlife?
Sebaceous hyperplasia mostly shows up after your 40s, and more often in people whose skin has always run oily. As we age, sebaceous glands respond differently to androgens, and cell turnover, the pace at which old cells get replaced by new ones, slows down as well.
There are also reports linking it to skin with prolonged sun exposure and to people on long-term immunosuppressants. It can run in families too, so if a parent or sibling has similar bumps, it's worth keeping an eye on your own skin.
Put simply, in your twenties, worn-out cells get swapped out quickly and the space stays tidy. As you age, that cleanup slows down, and older lobules just keep accumulating in place. That's exactly why these bumps become far more noticeable after your forties than they ever were in your twenties.
Could Many Bumps at Once Mean Something Else?
Most cases of sebaceous hyperplasia are nothing to worry about on their own. It's a common change that brings one or two bumps, or at most 5 to 10, as the face ages.
Very rarely, though, a young person develops an unusually large number of sebaceous tumors spread across the face and torso all at once. This pattern has been linked to a hereditary condition called Muir-Torre syndrome, which also carries a higher risk of other organ cancers, including colorectal and endometrial cancer. If you have a family history and notice an unusual number of sebaceous lesions, it's worth getting checked by a dermatologist just to be safe. That said, this scenario is quite rare, and in the vast majority of cases, age and skin type alone explain what you're seeing, so there's no need for excessive worry.
How Does Electrocautery Clear It?
Since sebaceous hyperplasia is almost always a harmless, benign change, there's no medical need to remove it. But if it bothers you cosmetically, electrocautery, which uses heat from an electrode to cauterize the tissue, can clear it away.
A fine electrode carries a high-frequency current that generates heat, and that heat coagulates only the enlarged lobule tissue. Lightly cauterizing around the follicle opening lets the swollen lobules collapse, flattening the bump.
Think of it as letting the air out of a balloon-like cluster of tissue, except instead of a needle, heat does the job. Since it's not a puncture but a gentle cooking and setting of the lobule cells, there's no risk of the wound reopening or bleeding, and the tissue simply shrinks quietly.
Lasers, including CO2 and erbium:YAG lasers, which precisely burn or ablate tissue, work on the same principle and can achieve the same result. When the bumps are numerous and widespread, a low-dose oral retinoid such as isotretinoin, a vitamin A derivative, is sometimes used to shrink the sebaceous glands themselves.
Right after treatment, a small scab forms over the cauterized spot, then falls off naturally within 5 to 7 days, revealing smooth new skin underneath. Because the cauterized lobule tissue doesn't regrow in that exact spot, the same bump reappearing in the same location is rare.
Why Does It Come Back, and How Should You Manage It?
Spots cleared by electrocautery or laser tend to stay clean, but the real issue is that new bumps often appear elsewhere. The procedure removes lobules that have already enlarged, it doesn't change the underlying tendency of your glands to enlarge again.
Since the underlying causes, oily skin, sun exposure, and age-related hormone shifts, are all still in play, similar bumps can appear in other spots over time. So rather than expecting one treatment to be the final word, it's more realistic to think of this as clearing new bumps every 2 to 3 years, as they show up.
Clearing the bump on your forehead, for instance, won't stop new ones from forming on your temples or cheeks. It's a bit like pulling one weed, only for another to sprout nearby, since the underlying tendency in your skin hasn't changed.

- Why you shouldn't force-squeeze them: Squeezing brings up nothing anyway, and forcing it can leave marks and scarring on the skin.
- Why a sudden increase warrants a checkup: In rare cases, similar-looking bumps can be confused with other skin conditions, so if several new ones appear in a short span or look unusual in shape, it's safer to have a dermatologist take a look.
So sebaceous hyperplasia isn't a condition that demands urgent treatment. It's enough to recognize it, tell it apart from acne, and clear the spots that bother you whenever you feel like it.
